Need Help Creating Custom Report

Hello,

Ours is a virtual consulting business and I charge our customer's cards manually on the Square website and I need help creating a custom report for use offline that will display:

1) The transaction date.

2) The customer name.

3) The gross sale amount.

4) The net deposit amount after fees and service charges.

5) The transaction fee.

6) The service charge.

Any help would be greatly appreciated.
