Integration of form with payment and payment button

Our organization had a need to allow people to complete a form and then make a payment. It would be wonderful to have these two processes integrated into one step with one submit button. We tried using JotForm to do that, but we kept getting a message that payment was not secure and that was a completely unacceptable option.

The work around was to list membership levels; and then make a form requiring the customer to press submit twice.

I'm going to direct you to the page on our weebly site, so you can easily see what we are trying to accomplish. A simple drop down menu with payment options at different levels on a custom form would be great.....and much better than having to go outside of weebly to accomplish this task.
