How can I take a payment without access to computer or smartphone?

Does Square have a form where I can write down credit card information, for later input, and give customer a receipt for the transaction?

No, there is no offline form. You can write down the card number, name, 3 digits from back, zip, phone number and run it later but you would be 100% responsible for fraud, over the limit credit cards, bad numbers written down etc.

you can do offline processing in app too.

so it is not worth the risk to not process it offline in my opinion. Way too many options for you as the seller to loose inventory without payment.

Thanks VanDalkerFarms! You made some good points! You could do it in the old days with a 2-3 part invoice slip and a manual device to capture the raised lettering. But still would have the liabilitity concerns. Thanks again..:)
