When cleint Book's a service they also need to enter contact informtion.

Can I edit those fields?

Hey @abe1! When a customer books an appointment online they have to enter their email address and phone number.

We're you looking to collect other contact information?

Hi Chad

I am a mobile service starting with Square Appointments. I need to know certain information (bicycle type, bicycle tire size, shifting type, and about 8 other questions) when they schedule the appointment. Is there a way to automatically gather this information when the customer books? Does Square integrate with any kind of forms that can be automatically sent to gather this information?

Hey there @COD76177 ,

There is currently no way to automatically collect that information outside of using notes. We do have contact form integrations on the dashboard that could help with that.

Thank you for the response. I am utilizing Square appointments only. How would a custom form integrate with appointments? I created a jot form with the questionnaire. How would it integrate?
