Setting Up Payroll: Getting past "Withholding Permit Number"

I'm in the process of setting up Square Payroll for my business in Iowa but am having trouble getting past the screen that needs my EIN, etc. The area I'm
getting stuck on is the IA withholding permit number. When I put in my
EIN, it automatically fills in the permit number box, but has an extra 3
digit space that it does not fill, so when I try to move past this screen
it won't let me. I've looked through my materials from the IA Dept of
Revenue and I don't see anywhere that I'd have an extra 3 digit number as
part of a permit number. No matter what number I start to type in to this space it shows it as incorrect. Anyone else have issues with this?
