Currently we don't have the ability to request additional information at check out or add a custom form. In the interest of being transparent, I can let you know this is not on the road map for the Square Online Store.

As a restaurant operating during covid times, we've shifted our concept slightly. So, now our customers order on their phones from their table and we bring them their order. The problem is that many customers forget to enter their table number in the "Note to Seller" section and it's become a hindrance to running our business. If we could just have an option to include a required field at the end of checkout for customers to enter their table number, we'd be set! Any ideas on how to achieve that?

Adding mandatory fields at checkout isn't supported at the moment, but I recommend adding a text modifier to your items so parents can enter their child's name and group before adding to the cart.

Required fields at checkout is not supported as addressed in this post. However, i provide a workaround that might help.
