yes, a card reader can be plugged into or linked via Bluetooth to any device. It will use the account that the device is logged into.

My employee is trying to connect a card reader using the appointment app on her phone and it's not allowing her to select the card reader option, is there a permissions issue that needs to be changed?

Yes this may be due to his/her permissions. Take a look here at how you can make these changes by heading over to Permissions on your online Square Dashboard.
