I need to link a vendor application to my event item, how do I do this?

I am producing a festival next summer and want to sell tickets and vendor spots through my square online store. I need to be able to link or create an application for the vendor spots as well as my Liability Waiver, How do I do this? Is it even possible?

There isn't a way to attach your own form but Square offers several pre-made contact forms that are customizable so you can grab important customer info at checkout.
