Creating Custom Fields?

Hi!

Love square!

I need some guidance on a new project. I have a children’s book coming out and when parents click a square link to order I need them to fill out 2 custom fields.

1) Childs first name if they would like the book signed

2). School Name, so school can credit if using as a fundraiser.

What is the easiest way to construct these fields as I am creating the item?
