Why can't I create a Service?

I created my first service today. When I attempted to save it, I was prompted with "We’re sorry, but something went wrong." I then closed the screen, and started over again on a new window. Now when I click "Create Service" the window scrolls up with the error at the top of the screen, every time.

How can I resolve this?
