Is there a way to require a note to be entered when a discount is manually applied to an item in retail or restaurant? For instance, if one of my managers discounts an item for a customer, I want to know more detail about why. Or if they apply an employee discount, I want them to have to enter the name of the employee that received the discount.

Good idea but there isn't a way to require a note. Maybe Restricted Discounts with Team Management will work for you. It requires a 4 digit passcode from a manager to apply the discount to a sale. That way, you or your managers will be looped in on why a discount was applied.
