Can I create a contract that includes fill-in-the-blanks, and optional initials?

I'm a photographer, and am hoping to automate the contract/model release step. On my document I capture names and birthdates of family members (fill in the blanks). In addition I ask for permission to use the images for print, website and/or social media marketing. I need the client to be able to check off the media they are comfortable with, and not the ones they are not. Is there a way to include this information on a Square contract?

The other items in the contract just require simple initials. So no issues with those.

Right now you are unable to include fill-in-the-blanks on contracts but you are able to add initials and clauses. Thanks for reaching out to share your feedback.
