&lt;P&gt;That is a great question, however, unfortunately at this time that feature is not available for classes or events. You are able to provide this feedback on our &lt;A href="https://community.squareup.com/t5/Feature-Requests-Ideate/ct-p/Beta-Feature-Requests" target="_self"&gt;Square Ideate&lt;/A&gt; board where other Square Community users can vote on this. Are you planning on listing your classes on a Square Online site? If so, one workaround could be to add required text modifiers to the item, so when a customer makes a purchase, you see the information submitted.       <title>Intake Forms For Classes?</title>

      <description>&lt;P&gt;It seems I can only add intake forms to appointments and not classes. Is this something that will be added in the future?&lt;/P&gt;</description>
