Booking Questionnaire

Is there any way for me to add custom questions to by booking form to make it more personalized? There is a good amount of specific information i need for each individualized appointment so if i could add multiple choice questions or more short answers to the booking form, it would be incredibly helpful.

Hi @Austin88Ashby. Currently, this capability does not exist for online appointment booking. Normally, I’d suggest that you submit a feature request. However, just a quick review revealed at least three have been submitted on this topic. So, I’m going to provide those links and suggest that you follow those links and add your like/upvote them along with your helpful/constructive comments. Best of luck.

This link is on the subject of allowing modifiers attached to services to be displayed and used. This is helpful since online modifier sets allow something called a text box which is a free-format box that customers enter information into.

This link and this link are two that I found specifically requesting what you did.

No doubt there are others, and the admins just haven’t gotten around to merging them all together, which would be helpful @_Violet @MayaP

I’d recommend at least upvoting/liking the one that seems closest to your request, but it couldn’t hurt to like them all with the same comments.
