I can't find the Contact me form in the site anywhere. Please advise.

I added one by going to the EDIT SITE ---> Click the drop down menu at the top and then scroll to Add A Page under the Standard page section ---> scroll to the more section and click CONTACT US. You can customize it from there by adding questions from the right side menu.
